Safe Care Practice Psychiatry and Family Health PLLC is dedicated to providing high-quality psychiatry and family health services to our clients. To ensure a smooth and efficient operation, we have established clear policies and procedures for our clinic.
These guidelines are in place to guarantee a consistent and professional experience for our clients as well as our staff members. Please carefully review these policies and procedures and feel free to contact us if you have any questions.
All appointments must be made in advance by calling the clinic during business hours or request an appointment online through our website or emailing us. Clients are encouraged to schedule appointments in advance to ensure availability. Clients must arrive on time for their scheduled appointment. If they are more than 15 minutes late, they may be asked to reschedule. Same-day appointments may be available in urgent cases and will be scheduled at the discretion of the provider.
All clients must fill out a detailed intake form before arrival at the clinic. This form will include basic personal information, medical history, mental history and the reason for seeking treatment.
A government-issued photo ID will be required to verify your identity.
The intake form must be completed in full and signed by the client.
The clinic will review the intake form and determine if the client's needs can be met by our services. If the client's needs are outside of our scope of practice, they will be referred to the appropriate healthcare provider or ask them to seek another provider.
Clients must also sign a consent form agreeing to receive treatment at our clinic.
Any relevant medical records or previous treatment information should be provided by the client or their previous healthcare provider.
Clients under the age of 18 must have a parent or legal guardian present during the intake process.
If a client requires a translator, our clinic will provide one at no cost.
Our clinic values diversity and provides services to all individuals regardless of race, gender, sexual orientation, religion, or background.
All information provided by the client will be kept confidential according to HIPAA regulations.
Our clinic provides a safe and non-judgmental environment for clients to discuss their mental health or medical health concerns.
Progress notes will be taken during each session and kept securely in the client's electronic file.
Treatment will be tailored to the individual needs of each client, based on their goals and preferences, and will be reviewed and revised as necessary.
With the client's consent, we may consult with other medical professionals involved in the client's care to ensure comprehensive treatment.
Treatment plans will be collaboratively developed with the client and regularly reviewed for updates and adjustments.
Clients are expected to actively participate in their treatment and to be open and honest with their healthcare provider.
If a client is unhappy with their provider or treatment plan, they are encouraged to discuss their concerns with our clinic manager to explore alternative options.
Our clinic provides psychiatric and family health services only. Any other physical or specialty health concerns should be addressed with their primary care physician or that specialty physician.
We reserve the right to refer clients to other mental health or family health professionals if their needs fall outside of our scope of practice.
In the event of a cancellation, we require 24 hours’ notice. Late cancellations or no-shows may result in a fee.
Clients can cancel up to 24 hours before their scheduled appointments. Canceling within 24 hours of the appointment time will require a cancellation fee equal to the cost of the scheduled service.
The clinic reserves the right to reschedule or cancel appointments in the event of unforeseen circumstances, such as staff illness or emergencies. Clients will be notified as soon as possible and offered an alternative appointment time.
Clients can reschedule up to 24 hours before the appointment by calling or emailing us.
No shows will result in being charged the full cost of the service. Credit cards are kept on file and payment must be made before the time of the appointment.
Appointments will need to be rescheduled if more than 10 minutes late for a medication follow up and more than 15 minutes late for an initial psychiatric evaluation, Weight Loss Management and other appointments. If our appointment begins late because you arrived late, the session will end at the time it normally would have ended.
This is so that other patients may stay on schedule. If we begin late because the providers are behind schedule, the session will be of its normal duration, to assure that patient concerns receive full and proper attention. If you arrive after the scheduled session has ended, we can sometimes accommodate you, if an already scheduled appointment cancels or no-shows, but this cannot be guaranteed. Otherwise, the visit will count as a no-show.
Many insurance companies may require a referral for your visit to our office. Please make sure to contact your insurance company and/or your primary care physician to ensure that you do get a referral if it is required. Failure to obtain a referral, when necessary, may cause your insurance company to deny your claim.

Please let us know at the beginning of the session if we need to devote some of our time to discussing or filling out forms or write notes or letters.
Please fill out forms to the best of your ability prior to the session so that we may use our time most efficiently.
Letters, forms, record reviews, and interactions with outside agencies requiring significant attention outside of scheduled appointment times, will be subject to a fee of $50 per 30 minutes.
Please note that disability/benefit forms can only be completed if you are seen regularly by this practice for 6 months.
The cost to complete any disability/benefit form is $250. Other provider’s notes are $200.
A Release of Medical Records must be signed to have your patient records sent to another health care provider. If you are requesting that your records, be sent to another provider, we ask that you supply us with their name, address, and phone number. Requesting medical paperwork to be filled out will cost $150 to be completed.

For non-emergencies, please use the client portal to send a secure message to your provider for the quickest response. You can also call or email as well, but please note that all calls or email messages will be responded to in 24-48 hours. Calls/messages left during the weekend will be responded to on the next business day.
Clients will be given enough medications until their next appointment. However, due to certain circumstances, if a refill is needed before your next appointment, please send a secure message to your provider through the client portal.
Controlled substances are prescribed sparingly in this practice in an effort to keep you safe. They are also highly regulated by the Drug Enforcement Agency (DEA). Our prescribing of such medications is monitored, and providers are held accountable for prescribing practices. State laws require that additional measures be taken when prescribing controlled substances. We want our clients to know how seriously we take the prescribing of these types of medications.
We currently do not prescribe controlled medications in the state of ARIZONA, NEVADA and OREGON. We only prescribe control medication and only, when medically necessary, in the State WASHINTON and NEW YORK.
Please note we will not offer an early refill if your controlled prescriptions are lost/stolen. Please safeguard your controlled prescriptions. We will also not offer an early refill if medications are improperly used or not taken as prescribed.
If for any reason the medication is not helping you, please talk to your provider for a review and possible adjustments if necessary
Payment is due at the time of service unless other arrangements have been made in advance. We accept all major credit cards, Zelle and insurance payments. If using insurance, clients must provide their insurance information and be prepared to pay any co-pay or deductible at the time of service. We are in-network with many insurance plans. It is the client's responsibility to verify coverage and obtain anynecessary authorizations before their appointment. If a client's insurance plan does not cover the full cost of services, they will be responsible for any remaining balance.
For clients using insurance for medication management, please note that all co-pays/deductibles are due at the time of your appointment. For self-pay clients, payment for medication management is also due at the time of your appointment. For all other services, payment is due when you book the appointment. We require all clients on the medication management track and weight loss management to keep a credit card on file.
We are committed to protecting the confidentiality of our clients and their personal information. All staff members are required to adhere to HIPAA guidelines.
All client information and records are kept confidential and will not be shared without a written consent from the client.
Client records will be stored securely and will only be accessed by authorized staff members.
Exceptions to confidentiality include situations where the client or someone else is in danger, or if legally required by law.
We welcome feedback from our clients and encourage open communication.
Any concerns or complaints should be addressed directly with the clinic manager or written to the clinic's email address.
We will take all feedback and concerns seriously and strive to resolve them to the best of our ability.
These policies and procedures are subject to change at any time and will be updated periodically. Clients will be notified of any changes through our website or email or through our combined consent forms that will be sent to you to sign before scheduled appointments.
